COVID-19 Self-Certification Survey:
When District 15 schools begin to reopen, all staff, students and visitors will be required to complete a self-certification process prior to entering any District 15 building.
Parents/guardians will be asked to check their child’s temperature and answer questions to ensure their child is free of COVID-like symptoms. The electronic self-certification survey can be accessed via the Skyward mobile app or on the Skyward Family Access website. See below for instructions.

If your child has a temperature of 100.4 degrees or higher and/or has any of the following symptoms, your child should remain at home. Reminder: If you keep your child at home for any reason, please contact the school’s attendance line to report the absence.
COVID-like symptoms include:
- Cough
- Chills
- Shortness of breath or difficulty breathing
- Fatigue
- Muscle or body aches
- Headache (not due to a chronic condition)
- New loss of taste or smell
- Sore throat
- Congestion or runny nose (not due to known allergies)
- Nausea or vomiting
- Diarrhea
To help remind parents to self-certify their student(s) each morning, reminder text messages and emails will be sent at 6:45am.
IMPORTANT: If you have not logged into Skyward Family Access this year or have forgotten your password, please ensure you have access. If you have forgotten your password, click the Forgot Password button and/or contact your child’s school for assistance.
