Transfer Students

  • An out-of-state or out-of-country transfer student must present a health examination that was completed within one year prior to entry into an Illinois school on a comparable form. 

    Out-of-state forms are accepted only for students transferring into Illinois schools for the first time. The exam must have been completed within one year prior to the date of entry into an Illinois school and must cover all "required" elements as listed on the Certificate of Child Health Examination form. 

    Enrolling for the first time means that the student has never previously attended an Illinois school. The child may be coming from another state or country or may have been home schooled. The student would need a physical that was completed within one year prior to the date of entry. 

    A child who enrolled in an Illinois school, left the state and then returned to Illinois at a later time would only be required to obtain a new physical at the required grade levels: Kindergarten, 6th  grade, and 9th grade. 

    Any student enrolling for the first time in a public, private, or parochial school in the state of Illinois is required to present proof of an eye examination performed by a licensed optometrist or ophthalmologist.  

    A transfer student has 30 days upon entry to complete the health requirements.
