The Illinois Department of Public Health and the State Board of Education require a number of immunizations and exams for school attendance in the State of Illinois. 

Physical Exams: All students entering Preschool, Kindergarten, Sixth and first time students entering an Illinois school are required to have a current physical examination completed on the approved Certificate of Child Health Examination https://www.d15.org/Page/278.

It is a STATE MANDATE (not the local administration) that requires all health records to be in compliance before October 15th of the current year.  The state also requires that we exclude any child from school on October 15th if the health records above are not complete.  Parents are asked to submit the following health records to their students’ Health Office as soon as possible, if they have not already done so.
